Hi. First problem is fps_max. In 1.04 the lowest value is "30". Latest zblock sets fps_max to 0 or 60+, because fps_max may be use to become harder to hit (fps_max affect in data flow between client and server). I might be wrong (netcode is different), but its known Source engine problem (solved in css by zblock).
Second - Rates. Default server rates are: sv_mincmdrate 0 (but cl_cmdrate can be set only to 30 or more so '30' is real value) sv_minupdaterate 20 and sv_minrate 3500. There is nothing about rates in lo3.cfg so if server operator doesnt change it everyone can play on 30/20/3500 rates. So, can dev. team set default rates little higher (100/100/25000 - standard nowadays) and also all sv_min... comands (to hmmm 60/60/10000 ?). I know we will find proper rates in leagues cfgs, but this little change may improve our comfort.
